Shared Goals of Care | Ngā Whāinga Tauwhiro

The use of Shared Goals of Care (SGoC) is to facilitate the conversation with clinician’s, patients, and whanau, in exploring the patient’s values, the care and treatment options available and agree upon the goal of care for the current admission and give direction if the patient deteriorates.

As health service providers, a system-level approach to SGoC needs to be undertaken to ensure these discussions occur with patients and whānau and within the responsibilities of Te Tiriti o Waitangi. The SGoC principles described will help guide providers in doing this.

These discussions are best explored when a patient is well, for example as an outpatient or when planning treatment or investigations. However, during an unplanned visit SGoC should be discussed early, and before an episode of acute deterioration. This will then provide the basis for all clinical treatment plans. When implemented, SGoC provides less risk of a patient receiving unwanted or unwarranted treatments which is increasingly important if/when their condition changes.
